This pretty much exactly what Irma said in Sarah's dream. I think Sarah's problem was that she had gone through so much pain and social resistance to be accepted for who she was, and Irma, in Sarah's mind, was acting as if it was a joke. But now that she sees how serious her friend is about this, she'll learn to accept it.
